Reminds me of a copy of a gender selection form, where someone was probably overwhelmed by long words like "heterosexual" (6 syllables!), and wrote "straight" at the "other" option.

Seems like it's some sort of digital intake form for a healthcare organization. The answer related to speaking with a doctor had me curious and googling "Epic healthcare" showed that company with matching logo as a healthcare software company.

May be helpful information to share with a doctor but understandable to say no for anyone who wants to confirm their doctor is LGBT+ friendly/trustworthy first before disclosing details.
